TCDN Required Enrollment Forms (by program)

The forms below are separated into two groups; a group you are required to read to be able to sign the Parent Agreement/Consent Form, and a group you are required to return to us before your child’s enrollment may begin.

After reading these forms, you will be able to complete the TCDN Parent Agreement/Consent Form, which will be returned to us along with other required forms.

Please complete and return the below required forms as part of your child’s admission.  All forms require actual signatures.  We cannot accept typed names in cursive font, or initials.  We also will no longer accept photos of forms due to quality issues.

Preschool Program Required Forms Information

OST Program Required Forms Information:(BSC/AM &PM Explorers/NPASC/SASC/WASC)

  • TCDN Parent Acknowledge Form rev 5.23– This form acknowledges that you have read all the forms, including the Family Handbook, located on the website under family resources.
  • Enrollment Packet Information for Families 2024-2025 ost– All information on this form is required. Please do not leave any area blank. Addresses and phone numbers must be included for everyone listed as a person authorized to pick up your child, or as someone to contact in an emergency, this CANNOT be a parent. Health Insurance information and signatures at the bottom of the consent are required.
  • TCDN Child Health Report rev 6.19-This must be a full report and include a physician’s signature, the date completed, and immunization record dates and your child must have been seen by a healthcare professional within the last 12 months.
  • TCDN Special Care Plan Form Rev 6.19– This form provides TCDN with important information regarding your child’s medical and educational needs. If your child does not have any needs, please write N/A.  This form is required.
  • OST Family Participation Form 5.24
  • OST Getting to Know Your Child Form 5.2024This form should be included only if your child is new to TCDN or will attend a new program.  

Other Enrollment Paperwork:(If Applicable)

  • TCDN Medication Log 2019– If your child has an EPI Pen, inhaler,  or requires TCDN to administer medication while your child is in care, please fill out this forms and return it to the main office.  We will not be able to administer any medication without this form.  
  • Information for families with children who have IEP/GIEP – (Individualized Education Plan) or GIEP (Gifted Individualized Education Plan) – A copy of this Plan or the goal sheet should be returned if applicable. This will help classroom teachers be familiar with specific goals for your child, and often enables a site to be eligible for state grant funds. Please return with an x if not applicable to your child.

ACH Debit Payment Information:

The ACH debit service is a popular and convenient method to pay monthly tuition and is used by over 50% of our families.  It reduces the need for families to drop off or mail monthly tuition checks to the TCDN Main Office.  Families submit an ACH form to TCDN, and the monthly tuition is debited monthly from the account you provided on the ACH form.

Families who utilize this service will be charged a service fee of $1 per month.  The service fee will be deducted with the tuition between the first and the fifth of the month.

TCDN also accepts all major credit cards.  Currently, the service processing fee for the credit card is 2.85%
